This is an application for space, not a contract; submittal of an application is not a guarantee or confirmation of space. Full payment and certificate of insurance must be received within 10 days of submitting this form. Failure to submit all documents within 10 days may result in denial of rental space. If space is denied for any reason, payment will be refunded in full.
